- CMHT / hospital discharge team
- Referral route
- A community mental health team, hospital discharge coordinator or social worker refers, with a support plan and risk assessment. Section 117 aftercare funding may cover the support element.
- Eligibility
- Suitable where a person can live in shared or self contained housing with support, including people subject to section 117 aftercare. Rooms are not registered care and do not provide nursing.
